Update on Name Change Endorsement Mailing: Name Change Endorsement Forms will be mailed to all policyholders whose policies originated under the Phoenix Life and Annuity Company and Phoenix Life Insurance Company names. They will be mailed in stages beginning in April and throughout the rest of the year. Once you receive your form, it should be kept with your policy, contract or certificate.
