General

To cancel your policy please contact the Phoenix Service Department at 1-800-541-0171. Surrender forms are individually created for each policy by request and processed via phone only.

Representatives in the Phoenix Service Department can provide general information about the contract values, benefits, features, and charges associated with Phoenix insurance contracts. You should consult with your advisors for investment, tax, or financial planning advice to address your personal financial circumstances.

Life Insurance

Payments can be made through monthly bank draft or electronic funds transfer (EFT). Use the form below to set up payment via EFT. Premiums can also be made via personal check for quarterly, semi-annual or annual payment modes. Payments cannot be made via money order or credit card. We do not accept payments online at this time.

Annuities

There are different ways to withdraw funds from your annuity contract. If you would like to discuss your different options, please contact our Phoenix Service Department at 1-800-541-0171. You may also visit Service Forms to download the appropriate form based on state of contract and product type.

Please complete the Annuity Direct Deposit form and make sure to include a voided check for the new account.

To activate your income rider complete the Guaranteed Lifetime Withdrawal Rider Activation form. Please note, in most cases, the rider cannot be activated until 30 days from the issue date. This benefit can only be elected at issue.

First year required RMDs, should be taken from your existing qualified account prior to transferring the funds to Phoenix. An annual notification is sent to contract holders of qualified accounts once they have reached the age of 70 1/2.

Phoenix sends out tax information by the end of January each year. The Phoenix Service Department is prepared to assist you during tax season. We are able to explain your 1099R’s and send duplicate copies upon request. Form 5498 is sent out in mid-May. Please be advised we do not provide tax advice.
